How do I update a name or address in my LPA?

You must tell the Office of the Public Guardian (OPG) if either you or one of your attorneys changes name or address.

For a change of address, you do not need to send in any evidence of the address change.

If you or an attorney changes name, you must send in a copy of the proof of name change. E.g. marriage certificate or deed poll showing the new name. Make sure that you do not send in any original documents.
